[xml-h] Sanctioning the xhtml extension for use with application/xhtml+xm l

It's probably superfluous to mention it here, but for those that
missed it: http://www.xml.com/pub/a/2003/03/19/dive-into-xml.html has
some say about it, including making mozilla happy with the correct
xhtml+xml contenttype.
